​Do you understand the patient journey?

The patient journey describes all the sequential steps in providing a patient’s care. This may include clinical and non-clinical steps. Data analysis is necessary to evidence what is working, what is not and where the gaps are. Often, perception of why delays occur is not supported by data. Patients and carers should be given the opportunity to contribute to their own care plan. As part of an ongoing process, the voice of patients and carers need to be captured.​​​​​​​​​​​
