Social Impact Investment

​ The NSW Government is committed to delivering world-class healthcare to the people of NSW, including opportunities to enhance community palliative care and mental health care. Social Impact Investments were designed to gather evidence for novel outcome based contracting structures and service models in two areas:

  • Silver Chain Community Palliative Care Service Silver Chain Group provided community palliative care services in Western Sydney LHD from July 2017 to June 2024. This service has now transitioned to WSLHD and an evaluation is underway.
  • Resolve Program: Flourish Australia provided enhanced community care for vulnerable people with mental health conditions in Nepean Blue Mountains and Western NSW Local Health Districts (LHDs) from September 2017 to September 2024. The program has ended and a final evaluation of the service will be conducted.
