Community HeathPathways is an online clinical and referral information portal that helps health practitioners in primary care to make the right decisions, together with patients, at the point of care. Each clinical pathway is an agreement between local general practice, specialist services, and allied health practitioners about how to manage patients with a particular symptom or condition in their local context.
The pathways act as an everyday guide for health practitioners to manage a wide range of conditions and provide access to clinical management and referral advice. Pathways are evidence-informed, reflect local environments and aim to preserve clinical autonomy and patient choice.
To support SafeScript NSW, enhancements have been made to Community HealthPathways. This includes the development of the SafeScript NSW resource page and updating existing content to help health practitioners better navigate local pain, mental health or addiction services.
Clinical Pathways for GPs and Pharmacists have also been developed to support prescribers and pharmacists to treat patients who may be identified as at risk of harm by SafeScript NSW.
Hunter New England and Central Coast Primary Health Network (PHN) led the development of this HealthPathways content, which has been adopted and adapted by other PHNs to support health practitioners in their local areas.
