Privacy and Information Sharing

HealthOne NSW aims to create a stronger and more efficient primary health care system by bringing together general practice, community health and other health care services. The health professionals involved in HealthOne NSW work together in multidisciplinary teams to provide prevention, early intervention and continuing, comprehensive primary health care.  In order to provide this integrated care a patient’s health information will need to be shared.

The HealthOne NSW privacy resources, developed in conjunction with HealthOne NSW staff, provide a guide and resource for ensuring that HealthOne NSW clients are informed about the sharing of their health information.
 

HealthOne NSW Privacy Guidance for Local Health Districts

This document provides guidance for HealthOne NSW staff about the responsibilities around informing patients about sharing their health information.  This document also provides a draft script for the discussion with patients.
 

An important notice about HealthOne NSW 

This document is a patient leaflet about HealthOne NSW. It includes the NSW Health Privacy leaflet.  It is recommended that a copy of this document is kept on the patient record.

This document is a patient leaflet for patients who are enrolled in both HealthOne NSW and Connecting Care.  It also includes the NSW Health Privacy leaflet.  It is recommended that a copy of this document is kept on the patient record.
 

Request for withholding information from health care providers

This document is for use in the rare event that a patient objects to the sharing of their health information with other HealthOne NSW services and/or their partners.  The NSW Health Privacy Manual (Section 11.2.1.3) provides further guidance for dealing with this circumstance.

This document outlines the privacy obligations for providers of HealthOne NSW services who are normally external to NSW Health.
 

Privacy Manual for Health Information 

This document has been developed by NSW Health. It provides detailed operational guidance for health service staff to the legislative obligations imposed by the Health Records and Information Privacy Act 2002. The document outlines procedures to support compliance with the Act in any activity that involves personal health information.
