Diabetes mellitus

In NSW, diabetes represents a significant public health challenge that can lead to frequent hospitalisation and long stays for patients. The incidence of Type 2 diabetes increases with age.

People with Type 2 diabetes are at risk of developing complications including heart disease, stroke, kidney disease, peripheral vascular disease, retinopathy, neuropathy and lower limb amputation.

An evidence based model of care and guidelines have been developed by clinicians to provide optimal care pathways for patients to:

  • improve the in-hospital management of diabetes mellitus
  • enhance the patient experience and health outcomes
  • reduce hospital readmission.​
