The NSW Health Out of Hospital Care (OHC) Program (formerly known as the ComPacks Program) began in 2003 as a pilot project with 10 referring hospitals. The program expanded in 2018 with the introduction of the Safe and Supported at Home (SASH) packages and in 2021 with the addition of the End of Life (EoL) packages.
These services are critical to supporting discharges from hospital and preventing avoidable admissions as research shows that patients and their families/carers can face major barriers to accessing care in the community including long waiting times for services.
