Emily Cummins: We’d like to acknowledge the traditional owners of the land on which we meet today – the Gubragal people of Dharun nation – and pay our respects to elders past and present.

We wanted to improve the admission processes for frail, elder patients admitted to the emergency department through early identification, rapid, integrated and collaborative approach to their care.

Dr Danielle Ni Chroinin: ARIA provides streamline care for older patients, from early targeted identification in the emergency department through to early safe transition back to the community.

Feedback has been positive. Furthermore, ARIA’s integrated patient-centred model of care has translated into real reductions in length of stay for elder patients.
