Role of Patient Centred Co-commissioning Groups (PCCGs)

Local health districts (LHDs) and primary health networks (PHNs) and their partners form regional partnerships known as Patient Centred Co-commissioning Groups (PCCGs) that are jointly responsible for improving care for their communities.

PCCGs develop local care pathways and distribute available resources to address identified needs, joining up care for patients. They unite diverse organisations and professional groups around a single patient-focused vision for local health need and co-commissioning services. Through these partnerships, PCCGs leverage local resources and minimise waste.

PCCGs develop their own governance model and take responsibility for model design and determining target populations. They work with community representatives, organisations and professional groups to co-design a ‘care pathway’. To nurture innovation, each PCCG determines how they implement their care pathway.

NSW Health has established guidelines to support each PCCG in determining the:

  • type of services commissioned
  • costing of the care pathway
  • mix and skillset of support staff for care coordination
  • reporting mechanisms
  • development of quality improvement strategies
  • provision of financial incentives to providers.
