There are six partnerships currently in joint development or implementation phases:

Implementation phase

Western Sydney Care Collective

Value-based urgent care aims to reduce the number of low acuity conditions requiring “urgent" treatment presenting to Emergency Departments by providing sustainable alternative local patient-centred urgent care, at the right time and in the right place.

Cardiology in community aims to deliver integrated and coordinated care in the primary health care setting, with access to and support from acute and community care sectors.

Northern Sydney – Urgent care for frail and elderly persons

Uses data to identify frail patients at risk of requiring urgent care and proactively managing them to ensure they receive the right care, at the right time. A concierge service will coordinate care and link patients with services with higher value primary care involvement and rapid access to specialist support.

Murrumbidgee – Living well your way

Targets people diagnosed with Congestive Heart Failure (CHF) and/or Chronic Obstructive Pulmonary Disease (COPD) who have presented to hospital. The aim is to keep people with COPD and CHF healthier at home for longer by delivering primary care and community-based services aligned with best practice.

Western NSW/Far West – Living Better and Stronger

Targets poorly managed diabetes by providing patient-centred care with flexible, team-based models of care centred around general practice. Includes case conferencing virtually with specialists and allied health professionals, supported by practice nurses and care coordinators, to place the patient at the centre.

Joint development phase

Nepean Blue Mountains - Heart and Lungs Targeted Supports (HALTS)

Focuses on improving the quality and continuity of care for people living with Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF). Through a collective approach Nepean Blue Mountains will support people living with COPD and CHF on the journey to achieve their best health outcomes.

South-Eastern NSW/Illawarra Shoalhaven/Southern NSW

Targets people diagnosed with Chronic Obstructive Pulmonary Disease (COPD).
