Application for new private health facility

Within 14 days of receipt of an application for a new private health facility, the application will be checked and an acknowledgment letter issued, including requesting documents that have been omitted or incorrectly completed.

This process may repeat depending on the comprehensiveness of the application. See the "stop the clock" provisions for other exceptions.

Approval in principle

Following receipt of a complete application the following actions will be undertaken within 8 weeks:

  • consultation with relevant Ministry of Health branches or the Local Health District where applicable;
  • Fitness and Probity check against the applicant for the licence; and
  • publishing a notice advertising the proposed facility (statutory requirement) with time allowed for public submissions

An approval in principal with conditions will be issued once those actions have been completed satisfactorily. The applicant is then able to construct, fit out and staff the facility. When the facility is ready the applicant must notify that the facility is ready for commissioning.

Commissioning and issue of licence

Within one month of notification that a facility is ready for commissioning, officers from the Ministry of Health will inspect the facility. Following the commissioning inspection and receipt of all necessary policies, procedures and certification the licence will be issued within 14 days unless "stop the clock" exceptions apply.

Application for transfer of a licence

Within 14 days of receipt of an application for the transfer of a licence for a private health facility, the application will be checked and an acknowledgement letter issued including requesting documents that have been omitted or incorrectly completed.

This process may repeat depending on the comprehensiveness of the application. See the "stop the clock" provisions for other exceptions.

Assessment

Following receipt of a complete application the following actions will be undertaken within 8 weeks:

  • Fitness and Probity check against the applicant for the transfer; and
  • publishing a notice advertising the proposed transfer (statutory requirement) and allow time for public submissions.

An approval in principle with conditions will be issued once the above actions have been completed.

Transfer settlement and issue of licence

The applicant must notify the settlement date to the Ministry of Health. Within 14 days after confirmation of transfer and compliance with all conditions a licence will be issued.

Application for alterations and extensions to facilities and amendments to existing licences

Within 14 days after receipt of an application for extension or alteration to a facility holding an existing licence, the application will be checked and an acknowledgement letter issued including requesting documents that have been omitted or incorrectly completed. This process may repeat depending on the comprehensiveness of the application. See also the "stop the clock" provisions.

Consultation / approval to proceed

Following receipt of a complete application consultation will take place with the relevant branches within the Ministry of Health and the Local Health District if applicable. Once that process is complete and within 8 weeks from receipt of the complete application the applicant will given approval to proceed including a written plan approval for alterations and extensions. See "stop the clock" provisions for exceptions.

The applicant will then construct, fit out and staff the facility and notify that the facility is ready. Ministry of Health officers will conduct a commissioning inspection within one month from notification.

Issue of licence

The licence will be issued within 14 days after commissioning of the extensions / alterations and receipt of all necessary policies, procedures and certification.
