The NSW Government has introduced a $500 pre-IVF Fertility Testing Rebate forout-of-pocket medical expenses incurred by eligible persons for certain pre-IVF fertility testing (the rebate). See Apply for the Pre-IVF Rebatefor information on how to submit your completed rebate claim.
Note that this rebate will reduce to $250 from 1 January 2023 and will be available until 30 June 2026.
This Checklist is to be used by Specialists for the purpose of confirming the patient/s is eligible for the rebate. Specialists must refer to this entire checklist when completing a printed copy of Pre-IVF Fertility Testing Rebate Form with the patient/s.
The rebate form can only be submitted for the rebate by the female applicant (In identifying the female applicant NSW Health acknowledges people in the transgender community, or who are non-binary, may receive fertility testing but may not identify as women). The form must be accompanied by one receipt for out-of-pocket expenses. These expenses could relate to your consultation fee or tests received by her, her partner and/or gamete donor.
Alternately, the Female Applicant can claim the rebate using a receipt for an out-of-pocket expense incurred by a partner and/or gamete donor on a fertility test. In these circumstances the partner and/or gamete donor must also sign the Pre-IVF Fertility Testing Rebate form.
