The NSW Government has introduced a $2,000 Fertility Treatment Rebate for women who are undertaking IVF or another Assistive Reproductive Technology (ART) treatment in a private clinic and incur an out-of-pocket cost. This Checklist is to be used by the ART Provider who is providing the ART treatment for the purpose of confirming the patient/s is eligible for the rebate. ART Providers must refer to this entire Checklist when completing a printed copy of the Fertility Treatment Rebate Declaration Form [PDF] with the patient/s.
The rebate form can only be submitted for the rebate by the female applicant. The form must be accompanied by one receipt for out-of-pocket expenses incurred during fertility treatment.
NSW Health acknowledges people in the transgender community, or who are non-binary, may receive fertility treatment but may not identify as women.
