The aim of the project is to increase the cumulative discharge total of very long stay mental health patients (over 365 days) in Western Sydney Local Health District by at least 10% each six months on an ongoing basis for the period of the program.
It supports people with severe and persistent mental illness with stays in hospital for 12 months or more and works with the consumer and their family or carer to determine the best community-based living option.
The project worked collaboratively with inpatient and community mental health teams, facilitating a total of 110 transitions back into the community, equating to a combined length of stay of over 555 years, discharged from Western Sydney LHD mental health inpatient units.
