NSW Health is collecting your personal information for the purpose of administering health promotion programs, such as the Munch & Move and Live Life Well @ School programs. The details you provided will be securely stored in our Population Health Information Management System (PHIMS), owned and operated by the NSW Ministry of Health and our digital partner, eHealth NSW.
Personal information is defined as any information or opinion about an individual whose identity is apparent or can reasonably be ascertained. Any personal information supplied by you will be managed in accordance with the Privacy and Personal Information Protection Act 1988 (PPIP ACT).
This Privacy Collection Notice applies only to the personal information you provided to NSW Health Local Health District staff when contacted about our health promotion programs for either your early childhood service or school. This includes your name, role or position, email address and/or phone number.
It is not compulsory for you to provide your personal information, however in the absence of a point of contact it may be difficult for NSW Health to coordinate health promotion services with your organisation.
Only authorised users of the PHIMS system, who are NSW Health employees or their approved contractors, have access to view your personal information.
NSW Health will not disclose your personal information to anybody else unless required or authorised by law, such as by court order.
NSW Health maintains robust security measures to protect personal information stored in our systems from unauthorised access, disclosure, or misuse, in accordance with the Privacy and Personal Information Act 1998.
Personal information collected and stored in NSW Health systems is retained in accordance with the State Records Act 1998. Information no longer required for administrative or reference use will be securely disposed of to prevent unauthorised access or disclosure.
