Brokered care

What is brokered care?

Care provided by support workers is available for clients who need help to improve or maintain their health and to live as independently as possible​.

This can include having a support worker to come to their home to:

  • remind them to take your medication
  • help with cooking, shopping, cleaning and personal care​
  • take them to appointments
  • give carers a rest
  • rehabilitation​.

A case manager can apply for brokered care support.

Who is eligible?

Eligibility for brokered care differs according to the client's local health district and the services available.

Local health districts

Central Coast LHD, Hunter New England LHD, Illawarra Shoalhaven LHD, Mid North Coast LHD, Nepean Blue Mountains LHD, Northern Sydney LHD, Northern NSW LHD, South Eastern Sydney LHD, South Western Sydney LHD, Sydney LHD, Western Sydney LHD.

To be eligible an individual needs to

  • have moderate to severe HAND or other HIV related cognitive impairment (such as PML or Cerebral Toxoplasmosis) with significant functional impact
  • have a case manager (not from Adahps)
  • live in or receive health care in NSW
  • have applied for an NDIS or aged care package​.

Notes for referrers

  • the referrer must identify the tasks that the client needs help with
  • an application for NDIS needs to have been made and the NDIS number provided. Brokerage can be provided until other funding commences
  • Adahps will determine the functional impairment using WHODAS 2.0
  • the level of HIV related cognitive impairment will be determined by a neuropsychological assessment​.

Western NSW LHD, Far West LHD, Southern NSW LHD and Murrumbidgee LHD​.

To be eligible an individual will:

  • be HIV positive
  • have co-morbidities
  • have complex psychosocial issues
  • live in or receive health care in NSW.
