Young Mums, Dads and Bubs – A Collaborative Approach to Care 

This program has been developed in partnership with headspace, a non-government organisation (NGO) and the MNCLHD Child & Family Health Nursing (C&FHN) service. Commencing as a pilot in 2012, the aim is to improve access to a range of free multidisciplinary services for high-risk new mums under 25 years, the dads, babies and children, into a warm and welcoming environment at headspace.
 
New mums are referred to headspace following a home visit from the C&FHN(1) nurse to check on how they are managing after having their baby.
 
Consumer acceptability is very high with an average of six clients welcomed per day. In 2018-2019 numbers increased to 162 with most mums returning for support. The longevity of the program sustained over seven years demonstrates the success of this integrative model of maternal, child and family healthcare between two organisations as an excellent example of adapting service to need.
